Notice of Privacy Practices: HIPAA

The staff at Harrison County Community Hospital takes our role in patient privacy protection very seriously. Your medical record is yours, but it is our responsibility to protect it.

Only the patient, legal guardian or durable power of attorney can request copies of medical records and must do so in writing prior to any information being released.

Requests for copies of your medical record should be directed to Harrison County Community Hospital Health Information by phone, by mail, by fax, or in person.

To request records by phone, call (660)425-2211 ext. 279.

To request records by mail, fax, or in person, print and complete the following Authorization Form and submit by mail to Harrison County Community Hospital Health Information P.O. Box 428, Bethany, MO 64424, by fax to (660)425-8668, or in person at Harrison County Community Hospital 2600 Miller Street in Bethany.

The HCCH Health Information Department is open Monday-Friday, 8:00 AM-4:30 PM.

Some fees may apply.

Please note, radiology images may be obtained by contacting the Harrison County Community Hospital Radiology Department.
